PARticipation waiver

FULL COURT COED HOOPS – WAIVER & RELEASE OF LIABILITY

 PLEASE READ CAREFULLY BEFORE PARTICIPATING

By signing below and/or attending any Full Court Coed Hoops (“FCC Hoops”) event, you acknowledge and agree to the following:

 1. Assumption of Risk

I understand that participation in full-court basketball involves intense physical activity and inherent risks, including but not limited to sprains, fractures, concussions, permanent paralysis, and even death. Risks may result from my own actions or inactions, the actions or inactions of others, the rules of play, the condition of the premises or any equipment, or other causes, including risks not known or reasonably foreseeable at this time. I voluntarily participate at my own risk and accept full responsibility for my safety.

2. Facility Safety

I agree to inspect the facilities and equipment prior to participating. If I believe anything is unsafe, I will immediately inform FCC Hoops organizers and will not participate until the concern is resolved.

3. Release of Liability

I hereby release, waive, and discharge FCC Hoops, its organizers, volunteers, gym partners, sponsors, and associated facilities (the “Releasees”) from any and all liability, claims, or causes of action for injury, loss, or damage to person or property, including death, whether caused in whole or in part by the negligence of FCC Hoops or the Releasees.

4. Indemnification

I agree to indemnify, defend, and hold harmless the Releasees from any claims, demands, losses, liabilities, and expenses (including attorney’s fees) arising out of or related to my participation in FCC Hoops activities.

5. Medical Treatment

I authorize FCC Hoops to obtain emergency medical care if necessary and release all persons involved in such care from liability. I understand that I am solely responsible for any medical costs incurred.

6. Insurance

I affirm that I have adequate medical or health insurance or will be financially responsible for any medical expenses resulting from participation.

7. Code of Conduct

I agree to respect all players, referees (if applicable), and facilities. Poor sportsmanship, aggression, or unsafe behavior may result in my removal from the group or events without refund.

8. Photo/Video Release

I grant permission for FCC Hoops to use any photographs or videos taken during events for promotional purposes, including social media and the FCC Hoops website.

9. Broad Construction

This waiver is intended to be as broad and inclusive as permitted by applicable law. If any portion is held invalid, the remainder shall continue in full force and effect.

10. Acknowledgment

I have read, understood, and voluntarily agree to this waiver and release.
